I had a similar situation with my 8600GTS ... (Screen freezes or I get the BSOD, on XP and Vista).. It came down to ordering a new motherboard. My new board, the one now, is XFX Nforce 650I Ultra Conroe and no issues with it on XP and Vista.
I think it also had to do with nVidia drivers, that's where the .dmp files were pointing to. Tried all kinds, but none fixed it (before).
Edit: The old board was not a nVidia chipset either. I believe it was AsRock who made it... (hybrid board)
